Human in the Loop: Where Enterprise AI Still Needs People.

Human review works only when authority is explicit. Define permitted, approval-required and blocked actions, then give the reviewer the evidence needed to decide.

Separate three different kinds of oversight

Review before action, review after action and review by exception are not interchangeable. High-consequence steps need approval before anything happens; high-volume, low-consequence steps are better served by sampling after the fact.

Give the reviewer enough context to disagree

A reviewer who only sees a recommendation will rubber-stamp it. Show the inputs, the source of each fact and the rule that produced the suggestion, so disagreeing is as easy as approving.

Design the escalation path first

Most failures are not wrong answers; they are unowned situations. Decide in advance who receives an exception, how quickly, and what happens if that person is unavailable.

Measure the quality of handoffs, not just containment

A workflow that contains 80 per cent of cases but hands over the remaining 20 badly has moved cost rather than removed it. Instrument how complete each handoff is.
